The three evaluation forms

Running the reaction and learning forms inside an LMS means the responses arrive dated and attached to each learner, so your Level 1 and Level 2 data is collected automatically instead of chased by email.

The Level 1 reaction form questions

The reaction form kept short on purpose. Six rated questions and one open comment is enough to spot a weak session without exhausting the respondent. Rate each on a 1 to 5 scale.

How to use the pack

  1. Agree the success measures first

    Before the course runs, agree with the sponsor what a good result looks like on the job, not just in the room.

  2. Issue the Level 1 reaction form at close

    Capture reaction while it is fresh, at the end of the session, so response rates stay high.

  3. Build the Level 2 learning check into the course

    A short knowledge or skill check confirms people actually learned, not just enjoyed it.

  4. Schedule the Level 3 observation for later

    Give behaviour time to show. Have the manager observe on the job at 30 to 60 days.

  5. Tie it back to results

    Use the Level 4 prompt to connect the behaviour change to the business measure the sponsor cared about.

  6. Feed the findings back into design

    Review the scores, fix the weak module, and drop the questions nobody answered.

Do I need to use all four levels?
No. Many teams start with Levels 1 and 2 and add Level 3 behaviour observation for their highest-value programs. The coverage matrix helps you see where the deeper measurement is worth the effort.
Can I adapt the forms and questions?
Yes. The Word and Excel versions are unlocked. Rewrite the questions into your own language, change the rating scale, rename the programs in the coverage matrix, and rebrand it.
Do I have to fill the form three times?
